BLUE: Assessment Questionnaires Pack

This package provides you with a range of tools to inform your work with children and/or their families. These questionnaires cover the child’s journey through many aspects of their development, their family relationships and the current presenting difficulties. The documents are designed as such that you can easily type into them or print off to write in them. As the documents are in the Microsoft Word format, you are able to add/amend/delete questions so you are able to completely tailor documents that will meet your professional needs.

Please note these documents contain questions only - they do not tell you how to score or otherwise interpret the information you are gathering. The interpretation should be based within the range of your own professional knowledge and experience, which you should be practising within a regular supervision schedule.

You will receive these seven Microsoft Word templates in a Winzip folder. Winzips are a way of transferring large amounts of data to you. They are really easy to open, often just by pressing and holding the folder icon, or right clicking the folder icon (the action depends on the brand and type of device you are using). If you are unsure, instructions are easily available on Google.
